Not very familiar with the species, but does appear to be Gomphostemma. As informed earlier it is Gomphostemma, but species could not be identified precisely. I feel it may be closer to Gomphostemma pedunculatum Bentham ex J. D. Hooker as per Plant Illustration

Regarding leaf base, Flora of China says base broadly cuneate to subtruncate.
